Additional E-mail Address Request

This form is to request an e-mail address in addition to your firstname.lastname@csueastbay.edu address.  Your new address will be nickname@csueastbay.edu where "nickname" is the name that you specify.  If the address is already in use by another person, we will contact you for another preference.  Please fill out the form as completely as possible.  We will send your request to the appropriate technical support area, based on the information you provide.
